Hartford HealthCare COVID-19 Vaccination Clinics
Where: Southington Calendar House Senior Center, 388 Pleasant Street, Southington
Thursday, 10/21/21 from 8:30 a.m. to 11:30 a.m. -or-
Tuesday, 11/30/21 from 8:30 a.m. to 11:30 a.m.
Clinics are walk-in, no pre-registration is needed
- J&J, Pfizer, and Moderna
-We will be offering first and second doses, as well as boosters/3rd doses
-For those receiving dose 1, we will work with each patient to schedule them for their second dose at a clinic local to them
-For those receiving boosters/3rd doses, they will need to present their vaccination card with record of receiving doses 1 & 2, vaccine administration date, and lot number (*information below on how to obtain lost cards)

Please Bring to Clinic:
-Your insurance card
-Your COVID-19 vaccination record card (if you are receiving your second, booster or 3rd dose)
*If your COVID-19 vaccination record card has been lost, please obtain record through the following methods:
· Obtain via Hartford HealthCare's MyChart Plus patient portal. Log into MyChart Plus ( https://mychartplus.org/myChart/Authentication/Login ). Questions? Call Hartford HealthCare's Community Care Center at 860-972-8100.
· If you have an account log into VAMS: https://vams.cdc.gov/vaccineportal/s/recipientlogin?language=en_US and view or download your Certificate of COVID-19 Vaccination (for instructions visit: https://www.cdc.gov/vaccines/covid-19/reporting/vams/professional/QRS-access-vaccination-certificates.html or
· Call (860) 509-7929 to request your Official Immunization Certificate. from the CT Immunization Information System (CT WiZ) at the Department of Public Health. Vaccination providers are required to report COVID-19 vaccinations to CT WiZ.
